 Anonymous hackers broke into a server belonging to consultancy firm Booz Allen Hamilton and published a database containing some 90,000 military e-mail addresses and hashed passwords in what they have named Military Meltdown Monday

 A recent report (PDF) from the Treasury Inspector General for Tax Administration (TIGTA) found that the USA IRS has some serious problems with the security of nearly all of its 2,200 databases.

There has been a string of high profile security breaches this year – and the problems continue to threaten companies. It’s never too early to review your defences.
